More than 70% of cases ( cases) occur in developing countries ( in men, in women), and half the world total occurs in Eastern Asia (mainly in China). Age-standardised incidence rates are about twice as high in men as in women, ranging from 3.9 in Northern Africa to 42.4 in Eastern Asia for men, and from 2.2 in Southern Africa to 18.3 in Eastern Asia for women. Stomach cancer is the second leading cause of cancer death in both sexes worldwide ( deaths, 9.7% of the total). The highest mortality rates are estimated in Eastern Asia (28.1 per 100,000 in men, 13.0 per 100,000 in women), the lowest in Northern America (2.8 and 1.5 respectively). High mortality rates are also present in both sexes in Central and Eastern Europe, and in Central and South America.

8 Meta-análisis de quimioterapia en cáncer gástrico avanzadoA través de los años se han desarrollado decenas de estudios en cáncer gástrico metastásico (y los no resecables) tratando de encontrar la mejor forma de manejo para esta enfermedad incurable. Los estudios han sido muy heterogéneos y, hasta hace poco, no había una visión coherente sobre el manejo del cáncer gástrico metastásico. Con la publicación del meta-análisis de la Cochrane sobre quimioterapia para cáncer gástrico metastásico ya contamos con un soporte evidenciario que nos permite sacar conclusiones sólidas sobre este punto (Wagner AD, Grothe W, Behl S, Kleber G, Grothey A, Haerting J, Fleig WE. Se concluye que la quimioterapia incremente la supervivencia y el tiempo libre de progresión en pacientes con cáncer gástrico avanzado. Cáncer gástrico o de la unión gastro-esofágico localmente avanzado inoperable, recurrente o metastásico. Adenocarcinoma. Tumor HER2-positivo. Criterios de exclusión: Quimioterapia previa para su enfermedad avanzada o metastásica. Síndrome de mala-absorción o perdida de la integridad física del tracto gastro-intestinal superior. Historia de enfermedad cardiaca (ICC, FE <50%, etc). Disnea de reposo, secundaria a su enfermedad tumoral u otra enfermedad o pacientes con oxigeno terapia de soporte.
51 Desenlaces Primario: Secundarios:Supervivencia global (OS): Medida desde la aleatorización hasta la muerte por cualquier causa. Secundarios: Supervivencia libre progresión (PFS), Tiempo a la progresión, Tasas de respuesta (ORR), Duración de la respuesta: Evaluación tumoral cada 6 semanas. Seguridad: Eventos adversos (NCI-CTCAE v3), parámetros de laboratorio, creatinina: Una vez por ciclo. Ecocardiograma: Cada 12 semanas.
