41 ESTUDIOS DE INTERVENCIÓNDiversos estudios, pequeños: calcioantagonistas, IECA, diuréticos, betabloqueantes, nitratos. Resultados NO CONCLUYENTES o CONFLICTIVOS ACCESS (342 pctes): candesartán vs placebo iniciado en el día 1 post-ictus (+ tratamiento ev rescate s.p.; una semana). Misma disminución de PA en ambos grupos: Menor mortalidad y episodios vasculares recurrentes al cabo de un año en el grupo de candesartán. CHHIPS (179 pctes). Labetalol y lisinopril vs placebo son antihipertensivos efectivos en la fase aguda del ictus, sin incremento de efectos adversos, pero no se observaron diferencias significativas en el objetivo 1ario (muerte o dependencia a las 2 sem), aunque sí una menor mortalidad a los 3 meses. SCAST (2029 pctes). Candesartán vs placebo 7 dias post-ictus. PAM 5 mmHg inferior en grupo candesartán. A los 6 meses de seguimiento, la PA fue de 143/81 mmHg en los dos grupos, y el objetivo 1ario de ictus, IAM o mortalidad CV fue de 120 episodios en el grupo asignado a candesartán y de 111 en el grupo placebo (N.S.)

50 RECOMENDACIONES GENERALES DEL MANEJO DE LA PA EN LA FASE AGUDA DEL ICTUSNo evidencias de que la modificación de las cifras de PA sea beneficioso en la evolución de los pacientes. Tampoco hay evidencias de que el tratamiento farmacológico sea perjudicial, salvo que induzca cambios bruscos. Si es necesario tratamiento: evitar descensos superiores al 25% de los valores iniciales en las primeras 24h (15-25%). Control estricto PA primeras 24h. Monitorizar evolución neurológica Pacientes previamente hipertensos: Reinstaurar tratamiento antihipertensivo a las 24h
