Medicina personalizada en la EIICSupondrá : Tendrá : Mejora en el manejo Repercusión Terapia óptima Sin efectos secundarios Mejora adhesión Reducción costes &tiempo de ensayos clínicos Cambio énfasis – de tratamiento prevención Reducir pérdida de medicamentos suprimidos Reducir el coste global de la salud pública select optimal therapy and reduce "trial-and-error" medicine; reduce adverse drug reactions; improve the selection of drug targets; increase patient compliance with therapy; reduce the time, cost, and failure rate of clinical trials; revive drugs that failed clinical trials or were withdrawn from the market; avoid withdrawal of marketed drugs; shift the emphasis in medicine from reaction to prevention; and reduce the overall cost of healthcare.
I want to stress the ‘bedside’ in the bench-to-bedside,” he says. “In fact, I want to turn that paradigm around, from patient care to mechanism of drug action.”
The SNPs used in DTC genetic scans come straight from the journals. In general, the companies scour the literature for papers that meet their criteria to ensure the study was up to snuff. At Navigenics, co-founder Dietrich Stephan says the criteria include checking the power of the study, and how the cohorts were selected and characterized. "The most important thing is that it has to have been replicated a number of times in independent cohorts," Stephan says. 23andMe and DecodeMe follow similar protocols to make sure that the study doesn't diverge too far from the "standard recipe," as 23andMe scientist Brian Naughton puts it. "[If] they start doing something weird, that's a good indication that they are trying a little too hard [to find an association]," adds Serge Saxonov, another founding scientist at 23andMe. The vast majority of papers that the companies consider come from GWAS. "We are focused on genome-wide association studies primarily, and then look to see if there's wide replication or not," says Decode Genetics' chief scientific officer, Jeffrey Gulcher. Decode also conducts many of its own studies into disease associations.

Distribution of True Genetic Relative Risk of Disease for Three Subjects, According to Their Current Estimated Risk and the Number of Undiscovered Risk Alleles for the Disease Distribution of True Genetic Relative Risk of Disease for Three Subjects, According to Their Current Estimated Risk and the Number of Undiscovered Risk Alleles for the Disease. Panel A shows the genetic relative risk of disease for three subjects with differing estimated risks with the assumption that there are 200 undiscovered independent risk alleles for the disease, each with a frequency of 25% and a multiplicative relative risk of 1.1. Panel B shows the same scenario with the assumption that there are 400 such undiscovered risk alleles. Risks are relative to the prevalence in the general population. The dashed line represents a relative risk of 1.0, the population median risk. Kraft P and Hunter D. N Engl J Med 2009; /NEJMp

Russell, Elaine R. Nimmoand Jack Satsangi, Figure 1. IBD susceptibility loci and subchromosomal regions identified in genome-wide scans using nonparametric linkage analysis Concordance data in twins (36% for CD and 16% for UC, 4%for bothCDandUCfor monozygotic and dizygotic twins, respectively) and multiplex IBD families (relative risk to first-degree relative of proband ≤35 for CD and ≤15 for UC) have provided strong epidemiological evidence for a genetic contribution to IBD susceptibility (5). These observations led to the application of nonparametric linkage analysis in 11 genome-wide searches for regions of the genome involved in IBD during the 1990s (Fig. 1) Am J Gastroenterol 2007;102:2820–2831

Generation and regulation of TH17 responses in the intestine. Under the influence of microbial-derived stimuli and ATP, lamina propria (LP) CD70high and/or CD103- DCs induce TH17 differentiation through the production of TGF-β, IL-6, and IL-23. Production of TGF-β by LP cells also promotes the anti-inflammatory Foxp3+–induced T regulatory cells (iTreg). The balance between TH17 and Treg cell development is favored toward Treg cells on the influence of RA produced by LP and Peyer patch CD103+ DCs, LP macrophages, and IECs. Besides RA, TSLP expressed by IECs and the cytokine IL-25 were reported to suppress TH17 development in the intestine. 22 Genomic Medicine ‘Grand challenges’ in the translation ‘Grand challenges’ in the translation of genomics to human health Geoffrey S Ginsburg Recently, Scheuner et al (J Am Med Assoc 2008; 299: 1320– 1334) carried out an extensive meta-analysis of 68 studies seeking to integrate genomic medicine to the clinical management of chronic diseases. The study posed the following questions. What are the outcomes of genomic medicine? What is the current level of consumer understanding about genomic medicine. What information do consumers need before they seek services? How is genomic medicine best delivered? What are the challenges and barriers to integrating genomic medicine into clinical practice? Dr GS Ginsburg is Director of the Center for Genomic Medicine, Duke Institute for Genome Sciences & Policy, Duke University Health professional and public education Building the evidence for clinical utility Cost uncertainty The need for an integrated approach European Journal of Human Genetics (2008) 16, 873–874
