5 Tufts Freedom Health PlanWDH offers one preferred provider option to employees under the Tufts Freedom Health Plan No requirement to pick a primary care physician No referrals required for specialist visits 3 Tiers of coverage levels are offered depending upon where services are provided. Deductibles, coinsurance and out of pocket maximums are different depending upon the tier where services are provided. Tier 1 = Granite Health Hospital Network of Providers* Tier 2 = Tufts Health Freedom Network Providers and PHCS Providers Tier 3 = All Non-Participating Providers If you are enrolled in medical, you automatically have pharmacy coverage through Envision RX. If enrolled in medical, you automatically have Vision Discount Plan through EyeMed. *if you live greater than 35 miles from all 5 of the Granite Health Hospitals; Catholic Medical Center, Concord Hospital, Lakes Region General Hospital, Southern NH Hospital and Wentworth-Douglass Hospital, all care at a Tufts Plan participating hospital closer to your home will be considered as tier 1. Massachusetts General and Brigham and Women’s Hospitals are also Tier 1. Providers with primary privileges at Granite Health Hospital are Tier 1. If primary privileges are at a non-Granite Health Hospital, they might not be considered Tier 1 providers. -1-

6 Tufts Freedom Health PlanCalendar Year Deductibles Once a deductible has been met, services will be covered at the following co-insurance rates: Tier 1 = 85% Tier 2 = 70% Tier 3 = 50%

7 Tufts Freedom Health PlanOut of Pocket Maximums Deductibles, co-insurance and copays are credited toward the out-of-pocket maximum. Once the out-of-pocket maximum has been met, covered services are paid at 100% within that Tier.

9 Health Reimbursement Account (HRA)As part of your medical benefit, Wentworth-Douglass Health System automatically provides you and your covered dependents with an HRA to help offset the first portion of your deductible. These funds may be used throughout the plan year for eligible medical, prescription, dental and vision expenses. You will want to use these funds before paying any expenses out-of-pocket. In addition to these initial HRA funds, you will have opportunities throughout the year to earn additional HRA funds if you choose to participate in TakingCare, WDHS’ optional Wellness Program.

10 TakingCare Wellness Benefit Eligible Employees Enrolled in MedicalWDH supports employee wellbeing. One of the ways we support healthy behaviors is by providing employee wellness dollar funding into an HRA. TakingCare Wellness provides quarterly incentives for participation in the program as follows: Total Annual Calendar Year Potential = $500* *Full year, prorated for eligibility that begins after quarterly incentives are held. All program answers are confidential and handled by a third party. 11 Northeast Delta DentalWDH offers two dental plan options through NE Delta Dental -2- Service Core Plan Caps + Crowns Deductible None $25/$75 Plan Year Maximum $1,000 $2,000 Cleanings and exams twice per cal year 100% X-rays, bitewings once per cal year X-rays full mouth panoramic once per five calendar years X-rays of individual teeth as necessary Fluoride once per year to age 19 Space maintainers, initial appliance only and all adjustments within 6 months of installation Sealants on permanent molars, once in a 3-year period per tooth, for children to age 19 Fillings, surgical and routine tooth extractions, root canal therapy, treatment of gum disease, periodontal prophylaxis (cleaning), repair of removable denture to its original condition and emergency treatment 50% 80% Orthodontia No 50% to $1,500 max per person lifetime Bridges, partial and complete dentures, rebase and reline, crowns, onlays and dental implants 0% May use any dentist but premier and preferred network providers have greater discounts.

12 Life and Disability Group Term Life:WDH provides group term life and accident coverage to benefit eligible employees at no charge. Employees working scheduled hours - $10,000 Employees working 24+ scheduled hours - $50,000 Long Term Disability: WDH provides employees scheduled to work 24+ hours per week with Long Term Disability coverage: Replaces a portion of your income if approved, and you are absent for more than 180 days. Benefit is equal to 60% of your base annual salary tax free to a maximum monthly benefit of $12,000. You are taxed on the value of the premium but receive the benefit tax free.

13 Earned Time *Full time accrual. Part-time is prorated.Earned time is a program that combines all the traditional paid time off benefit of vacation, sick time and holiday into one single category. All benefit eligible employees begin to earn ET immediately. 14 Voluntary Benefits Short Term Disability:Employees scheduled to work 24+ hours per week are eligible to enroll in Short Term Disability coverage which replaces a portion of your pay for up to 26 weeks if you are not able to perform your own occupation. Option 1: Disability income equal to 70% of your basic weekly earnings up to a maximum biweekly benefit of $6,000 beginning on the 1st day of an accident or 16th day of an illness; Option 2: Disability income equal to 70% of your basic weekly earnings up to a maximum biweekly benefit of $6,000 beginning on the 1st day of an accident or 46th day of an illness. Option 3: No coverage Supplemental Life Insurance: Additional life insurance of 1x, 2x, or 3x your salary up to $500,000 Guaranteed issue up to $350,000 upon first eligibility Evidence of insurability required for amounts between $350,000 -$500,000 Dependent Life Insurance: Spouse $15,000 Children $2,500 per child

15 Voluntary Benefits Flexible Spending Accounts (FSAs): Flexible Spending Accounts (FSAs) provide employees with an opportunity to pay for medical expenses for themselves and their eligible dependent with before tax dollars and provide an opportunity to pay for dependent care (child care) expenses with before tax dollars. Healthcare FSA Dependent Care FSA Annual EE contribution $2,550 $5,000 Deducted on a before tax basis Each paycheck To be used for: Un-reimbursed medical, dental and vision related expenses (allowed by the IRS) for you and your eligible dependents Child care expenses for dependents under the age of 13 when provided by an eligible provider. Examples Deductibles, copays, coinsurance for medical, dental and vision Eyeglasses, RX, Orthodontia… Licensed child care center, private day care, nannies, care for disabled dependents

16 Other Benefits Employee Assistance Program: Confidential counseling and referrals for limited time. Employee Referral Program: Bonus offered for eligible employment referrals subject to plan offerings. Tuition Assistance: WDH provides $3,000 each calendar year to regular full-time or regular part-time employees scheduled to work 16 hours or more per week. Must complete one year of service prior to start of course and receive a grade of C or better or Pass in a pass/fail course. Identity Fraud: Free insurance policy that provides financial assistance to those dealing with expenses associated with identity fraud. Adoption Assistance: WDH assists employees with expenses related to adoption of children up to $2,000 per child adoption under the age of 18 who is not a relative or step child. Community Volunteerism: WDH will match up to 4 hours of non-productive matching pay when an employee volunteers on their own time to other not-for-profit agencies. The Works Family Health and Fitness Center: Eligible for discounted membership fees. WDH discount of 25% for Wentworth Douglass medical bills and 20% quick payer discounts are available. WDH discount applies to hospital and employed physician based services. Please refer to Policy LD-36 and Policy FSPA-10 for more details.
