Almost 20% of the patients in our registry have CHD. Most of them are Eisenmengers but we also follow a significant number with closed defects.

It is 100% in troncus arteriosus but the frequency of this lesion at birth is very low. On the other the percentage of patients with pulmonary vascular disease in simple defects such as VDS, ASD, and PDA does not appear to high but the frequency of these lesions is high, Accordingly these are the patients that we see more frequently in our clinics and therefore I will focus the rest of my talk in this population

No difference between the survival of patients with Idiopathic PAH and that of the one hundred eleven patients with PH associated to congenital heart disease, and again…The survival of those with shunt closure was very poor. These patients went to repair at a median age of 0.6 years In the majority of them the PH persisted after the surgery and in four out of the seventeen patients PAH developed after a mean of 1.5 years. Patients with PAH-CHD after shunt closure (n = 17) underwent surgery at a median age of 0.6 years (range 0.2–7.6). In 13 of these patients, PH persisted after cardiac surgery, in the remaining PAH developed after a median of 1.5 years (range 0.9 –7.9)

In this report by doctor Engelfriet from the Euro Heart Survey, there were tree hundred and seventy seven closed ASDs and two hundred and seventy five closed VSD out of almost two thousand patients with CHD and, as we saw, PAH was present in 12% and 13%, respectively.

Heart 2007; 93: Long-term survival In the figures from this report we can see that both in ASD as well as in VSD, patients with closed defects and PAH have a lower survival expectancy than in those with a similar open defect or with a closed defect but without PAH. However, a survival rate above 90% at five years in these adult patients is not as bad as it was shown in children ASD VSD

I just did a preliminary analysis of fifty one of our patients with corrected CHD Most of them adult women with a late closure of the defects and most of them with ASD and PDA with severe residual PH and a significant proportion of the were included in the trials of modern drug therapy, and In this figure we see the survival after closure and in this other the survival after enrollment into our registry. As in the Euro heart survey, a survival above 90% at five years is not bad and certainly better than that reported in children

55 Conclusiones En pacientes con defectos simples la severidad de la HP residual y la respuesta al tratamiento esta modulada por edad, tipo y tamaño del defecto, sexo, Qp/Qs, y quizá otros factores Por ahora, la corrección temprana es el mejor camino para evitar la complicación de HAP Además de las variables hemodinámicas convencionales, debemos buscar otros marcadores de Enf Vasc pulmonar (CEC ?) El impacto de la terapia HAP-específica en el desenlace debe valorarse en estudios prospectivos y bien diseñados Allow me to conclude then that in patients with septal CHD the severity of persistent PAH after closure is modulated by…. Almost 20% of the patients in our registry have CHD. Most of them are Eisenmengers but we also follow a significant number with closed defects.

Patients older than 40 years old had a poor SV. In this study SV was also associated with the level of systolic PAP.

Eur Heart J 2006; 27: 1737 In this recent study of Diller and collaborators the authors did a retrospective and also a prospective analysis of the survival and they showed that although the survival expectancy for these patients is shorter compared with the normal population, a significant proportion of patients also lives to their sixties. For patients in FC II, the recommended drugs are endothelin receptor antagonists or sildenafil. For Sitaxsentan and Taladafil there is less evidence For patients in FC III, endothelin receptor antagonists, IV epoprostenol, inhaled iloprost and sildenafil are the first option.
