Practicum provides for the integration of coursework & field experience for competent skill-acquisition. 8 Securing your placementConfirm: Practicum site offer/acceptance and express your appreciation Obtain: Student Placement Form signatures; complete all info on form Submit: Documents to Field Education Office by the due date . Students may not start practicum until all documentation has been submitted. In person: Field Education Program office, Room 215 to Levi Heperi Or, scan & attach by Or, by fax: Required Documentation: 1) Completed Student Placement form (hard or electronic copy); and 2) Verification of Malpractice Insurance (upload into IPT) Placement is then Secured: Field Education office finalizes & approves. 9 Plus: Any Additional RequirementsVarious Site-specific requirements Website Handout: Site Specific Requirements Typical Medical site requirements Online HIPAA training is required through OUHSC for all OU students at medical placements. Completion is due 30 days from the start of practicum and login must be obtained from the Field Education Office, which will be ed to students near the start date.
10 Other Practicum PossibilitiesModified Practicum Request: Submit this request if the practicum needs to begin or end on dates beyond the Practicum Calendar. The request needs to be for significant and compelling reasons. Employment-Based Practicum Proposal (EBPP): Must represent new learning different from job responsibilities Must have different Field Instructor from job Supervisor Student must negotiate the practicum/employment placement Final approval determined by the Field Education Coordinator Website Handouts: Modified Practicum & EBPP information
