5 Section 125 Pre-Tax Benefit PlanSection 125 of the IRS code provides a Pre-Tax Benefit which allows you to have premiums for health, dental, vision, cancer, and medical/dependent care reimbursement deducted from your salary BEFORE Federal Income Tax is calculated. NOTE: As a result, no changes can be made to any section 125 plan payroll deductions during the calendar year, unless there is a change in family status.

19 Group Term Life Insurance – Basic Coverage Met LifeSpring ISD provides each eligible employee life insurance equal to 1.5 times their annual salary at no cost to the employee. You must complete the enrollment online and select a beneficiary.

20 Met Life Group Term Life Insurance – Optional Coverage on Employee- Eligible Employees have the option to purchase Additional Life insurance up to 5 times his/her annual salary, not to exceed $500,000. - The cost is dependent on your age (age-banded). Guaranteed Issue: $300,000 for new employees within 31 days of hire. **Not structured as an individual coverage for when you leave the district**

21 Met Life Group Term Life Insurance – Dependent CoverageYou can elect up to $100,000 in spouse coverage (not to exceed the amount of Employee coverage). Rate for spouse is dependent on employee’s age. Dependent child coverage is $2,000 for each child and the cost is $.32 per month Guaranteed Issue: $25,000 for spouse coverage and $2,000 for children **No double coverage is allowed. So if your spouse works for the district, you cannot take spouse coverage on him/her. Also, only one employee can cover the children at $2,000 total.

32 When do Benefits begin? Benefits coverage begins on the first day of the month following the first day of employment. Medical insurance can start on your first day of employment. 40 Add a New Dependent Please Note due to Federal Regulations under the Affordable Care Act, any spouse/dependent that you enroll in the medical coverage must have their legal name (as appears on their social security card) and social security number in the system for reporting and claim purposes. ALL EMPLOYEES must Elect or Decline Medical coverage for themselves and their dependents.

44 Beneficiary InformationIf you wish to have a Beneficiary that is NOT listed in your dependents Click Add Beneficiary, please make sure to fill out all Required Information. The option to split the amount is available, but it MUST add up to 100% of the total benefit amount. Once your Beneficiaries are designated Click Next at the bottom of the screen.

State Personal Days - Earn 1 day for each 36 days worked to a maximum of 5 days per year (State leave accumulate and transfer between districts). 49 Workers’ CompensationAvailable for an Employee injured while performing his/her assigned task. In order to benefit from Workers’ Compensation insurance, it is imperative that the Employee report any and all accidents at the time they occur. A first report of injury form must be filled out and submitted to their supervisor. Any Employee returning to work after being off for leave must submit a doctor’s statement to the Workers’ Compensation Office.

50 FMLA Must have worked as a full time employee for 1250 hours and atleast 1 year 12 weeks of unpaid job protection which prevents your employer from replacing you during your time off Can be used for yourself, spouse, children and parents ( if you are considered their caretaker) Forms are available on the benefits website

55 Spring Assistance for Educators (S.A.F.E.)- Cost is $24.00 per year to S.A.F.E. Membership allows employee to apply for small interest free loans to be paid back through payroll deductions. Membership does not guarantee a loan will be approved. Automatically enrolled every year. Please call benefits if you no longer wish to be a member during enrollment for S.A.F.E. Note: Remaining balance will be deducted from last check if an employee terminates employment and owes a balance.
