If approved, the Graded Death Benefit face amount will be the amount of coverage that the premium paid will purchase, and may be less than the Level Death Benefit Policy amount, but no less than $3,000. *For ages For ages 45 – 49 if they do not qualify for Level Benefit Plan they will be declined since the Graded Benefit plan is not available. 16 Final Expense Whole Life Application SubmissionPhone Call Interview The applicant will need to conduct a phone interview with Apptical. The available choices are: Agent calls with the client after the application is signed and submitted and interview is conducted. Agent calls during the sales interview and schedules the phone interview and adds reference number to application. Signed application must be received before interview can be conducted. Client receives an information sheet with call-in instructions. E-Signature Application is ed to the client for e-signature. Approval will include medical information authorization and bank information. Completed application forwards on to New Business for underwriting and issue. Apptical conducts telephone interview. 17 Final Expense Whole Life Policy DeliveryPolicy Delivery Options If the agent checks Mail to: Agent under Section 8, Acknowledgements, Declarations and Agreements, the agent is responsible for delivering the policy to the Insured. If Mail to: Applicant is checked, the policy will go directly to the applicant with no need for intervention. An is sent to the agent indicating the policy print had been mailed. Amended applications will not be active until the Amendment(s) are signed and returned. Any Conditionally Issued policy with an Amendment will be mailed to the agent. Failure of the applicant to sign and return the Amendment in a timely manner will result in the cancellation of the application. Apptical may use a telephone interview to assist the applicant in signing an “emendment” (over the phone policy amendment) to the coverage during the call. This will speed the process of policy issuance. Delivery Notice Every policy will have a Delivery Receipt contained in the Delivery Notice. Stress to the client that it should be signed, dated, and returned to New Business. Signed Delivery Receipt will confirm start date for 30 day Free Look Period. Confirmation Notices An is sent to the agent when the policy is mailed.
