1 The organization’s mission emphasizes a culture of Successful Aging within the community: "Christian Living Communities ministers to senior adults through a continuum of services and care that reflects Christian love, respect and compassion, and that enriches the quality and dignity of life for each individual." The mission statement is recited before all meetings and brainstorming sessions to remind ourselves why we come to work and on which to reflect when making a difficult decision. As a Christian organization, we believe that care is a key part of what we do in a compassionate, non-paternalistic way. Recently, residents were involved in selecting a new slogan for the community, and many suggested the word "care" because they felt everyone cares for each other like a family. We believe in empowering the residents to be engaged in the community. Holly Creek has many resident-led committees and interest groups that shape the daily experience for the community. The Thursday Thespians invite associates to participate in their performances. Holly Creek's Radio Station DJ's design their programming schedule and invite associates to contribute. A resident committee helps plan Holly Creek's diverse life enrichment calendar each month. This committee assists in achieving optimum participation. Associates are another important part of a culture of Successful Aging and they are immersed in the Masterpiece Living culture. 2014 DO use a typesize that can be read easily at a distance of ~4 feet or better. You do want a large crowd to develop around your poster, don't you? Think of 14-point type as being suitable only for the fine print and work your way up, (never down) from there. For text, 20-point type is about right (18 point in a pinch). Not enough space to fit all your text? Then shorten your text! Leadership Outreach Civic Engagement Residents and associates donate and distribute food at a local low-income senior housing community. The Resident Services Director teaches Chronic Disease Self Management Program there as well. Holly Creek Human Value Project, a group of residents share their human capital through various projects. They partner with Cottonwood Creek Elementary School students to provide reading and art support. Residents hand-craft wooden toy cars in a workshop at Holly Creek and these cars make their way to kids around the world in under-developed and war-torn regions. These volunteers have created over 100,000 of these handmade toy cars. Marketing Events Neurobics was presented as a sampling of a Masterpiece Living program as a six-week marketing event to engage prospects from the greater community in a Successful Aging lifestyle. We averaged 90 participants at each session. Marketing held a luncheon in which Dr. Landry presented on his latest book "Live Long, Die Short." This event brought 50 marketing guests into the community. The organization's annual fundraising event, Aged to Perfection was themed around the four components of Masterpiece Living. The silent auction items were organized by each component. Visible Statement of Support The mission statement is displayed at both community entrances and is recited at the beginning of most meetings. We have MPL logos on common room signs indicating areas of emphasis (SIPS). We also include MPL articles in the monthly newsletters. In the monthly ‘Chat with the Executive Director’, a portion is devoted to MPL discussion, including the Portrait and next steps. Every Monday, a SMILE Script is read at the stand-up meeting that includes MPL trainings and tidbits. A portion of the monthly All Associate Meeting is dedicated to MPL. Ten Minute Trainings are displayed on the Touchtown digital signs in the associate break rooms. Holly Creek's slogan is "No Limits”. Team Member Training All new associates attend orientation where they learn about Masterpiece Living. We discuss the history of Masterpiece Living, the four components of Successful Aging, the benefits of engaging in Masterpiece Living, lexicon changes that support Successful Aging, and what ageism, ableism, and paternalism mean. Residents present on the organization's core values and perform ageism skits. The Masterpiece Living Coordinator presents at the monthly All Associate Meetings. Weekly Wellness s are sent with messages of successful aging for associates. A mandatory annual training is held on multiple dates to allow for all associates to attend. Attendance is tracked and employees who do not attend are followed up with by department managers for one-on-one training. Culture Empowering Language Human Value is being used by residents instead of work experience. This is because a career does not define who we are as people. This is not the end-all-be-all of who we are. We are also using Older Adult instead of senior as this speaks to Successful Aging and also refers to the residents in their preferred manner. Senior Moment is a phrase we are using in conjunction with a resident speaking at the monthly All Associate Meetings. Residents come to speak to all associates to share their human value as it relates to the monthly core value of the organization. This is a term we are taking back and to which we are giving a positive connotation, instead of allowing it to be used in an ageist manner, to describe being forgetful. Associate is now used in place of employee or staff. This puts people on a more even level and they feel as though they are doing something because they want to, rather than because they have to. Resident/Member Engagement Successful Aging Programming Participation TGIF - A social happy hour where residents move from table to table to mingle with different people. They also dance to live music: 40% resident participation Wired World - A thoughtful look at the impact of current world and domestic events with a focus on what religious icons and scholars say in reference to them currently and historically: 25% resident participation Life Moves - An aerobic and social group exercise class encompassing elements of balance, strength training and stretching where the instructor and residents in the class quiz one another on current events and random trivia: 25% resident participation The Bible Jesus Read - An 8-week discovery of the Old Testament. These sessions explore 5 crucial segments of the Old Testament: Job, Deuteronomy, Psalms, Ecclesiastes and the prophets. Residents are led through a series of workshops then they discuss what they have learned: 25% resident participation Resident/Member Goals and Progress Lifestyle Reviews and Mobility Reviews are promoted via HCRK, the resident-run radio station, flyers, and the closed-circuit television channel. In 2014, Holly Creek held an event in conjunction with Dr. Landry's book tour. This resulted in a surge of Reviews this year. The Portrait was reviewed at ‘Chat with the Executive Director’. Mobility Madness is a large one-day event that resulted in 80 residents completing the Mobility Review. Application Process Holly Creek’s CSA Application was a two year process. Our first attempt in 2013 gave us baseline and we followed the guidelines from the review board and used them to formulate our 2014 CSA Goals. We set out to raise each CSA item score by at least one point. Process Formed a CSA team comprised of the following people. Each member focused on one or two categories as listed below. Executive Director - Leadership Assistant to E.D. - Outreach MPL Coordinator – Team Member and Resident Member Engagement Community Life Director - Purposeful Programming Entire Team - Culture. Communicate-We submitted weekly reports and had monthly meetings. These reports and meetings showed progress and barriers regarding each member’s category. The reports were discussed and disseminated by the team and a chosen course of action was agreed upon. We were always staying the course and correcting along the way. We always solicited input from the entire associate and resident population. Score-We kept updating each item’s new perceived score throughout the year. Application · Precision and brevity in essay answers. · Attachments are complete and pertinent to the question. · We asked residents and associates to review and edit the application prior to submitting. Team Member Engagement Team Member Successful Aging Opportunities We have many social events for team members which include Associate Appreciation Days, Christmas Party, Associate Picnic, the annual CLC Softball Tournament (won by Holly Creek...again), the Boat Regatta, and discounts to a local amusement park and movie theaters. Associates are also encouraged to assist with the bi-monthly resident TGIF happy hour parties. Online learning courses are offered monthly as a continuing-education requirement. There are also courses that associates sign up for voluntarily to further their professional growth. Scholarship opportunities are offered quarterly. HR s brain teasers to associates. As a part of wellness benefits, associates are offered a biometric screening and Live Well assessment through Kaiser Permanente at no cost. Thirteen associates participated in the annual CLC Biggest Loser competition and Holly Creek's team won the organizational contest. Forty associates participated in the 2013 Walk to Wellness campaign and use the indoor walking trail. As a wellness benefit, associates can get a discount to health clubs and Jazzercise. We offer an employee assistance program through Mines & Associates. Worklife Partnership assists associates in anything from finding housing to childcare and car repair. We also encourage un-interrupted breaks to promote a balance. Purposeful Programming Data Driven Programming Portrait data showed a high incidence of ER visits. One strategy is Chronic Disease Self Management classes that empowers residents to manage their chronic conditions independently through a successful aging lifestyle. We have seen a 13% reduction in ER visits. Low scores on the single leg balance from the Mobility Review lead to N'Balance and Tai Chi classes being added to the fitness schedule. Single leg balance time scores have increased by about 0.5 – 1.0 second. In 2013, State of the World worry score was 75% in the Portrait. The chaplains responded with new positive world event spiritual programs. The 2014 Portrait showed a State of the World worry score of 67%. These programs included Inspire, Wired Word and World Religions.
